JOB TITLE: Family Navigator
DEPARTMENT: Catholic Social Services - Community Based Services Division
REPORTS TO: Administrator
DIRECT REPORTS: volunteers and/or student interns as appropriate
Summary
Provides comprehensive social services to clients seeking assistance for a variety of social service needs with a concentration on individuals who are pregnant and families with young children under the age of 5 years. Conducts intake screenings and assessments, links to supportive services within the CSS network as well as within the surrounding community. Provides wrap around support as appropriate. Develops and facilitates regular family strengthening workshops for the area residents based on community and individual needs.
Principal Duties and Responsibilities:
- Conducts initial screenings to individuals to determine the level of assistance needed.
- Provides comprehensive one on one services to an identified caseload of families including intake and assessment, goal planning, linkage to appropriate community resources and facilitating regular meetings and visits.
- Develops professional relationships to provide the best advocacy, support and direction to ensure client self-sufficiency.
- Develops and facilitates weekly education groups on a virtual platform.
- Develops and hosts monthly community events for the larger community.
- Maintains current and accurate documentation of services provided and maintains organized client files in the Apricot database.
- Plans for and engages in weekly supervisory sessions with direct supervisor for the purpose of case consultation.
- Collaborates with CSS colleagues working together on community projects and attends regularly scheduled Family Navigation Meetings.
- Participate in regular staff meetings and CSS training/meetings and meetings/ training required by funders.
- Adheres to the requirements as outlined in the grantee response for funding provided by the Pennsylvania Department of Human Services (see attached)
- Other tasks and duties as assigned.
Knowledge, Skills, and Abilities:
- Knowledge of and orientation to the philosophy of Catholic Social Services as presented in the Mission Statement.
- Strict adherence to the teachings of the Catholic Church especially related to the position on abortion and contraception.
- Knowledge of crisis intervention techniques, child abuse reporting, de-escalation techniques and trauma informed care.
- Ability to maintain confidentiality regarding client information adhering to policies and procedures with knowledge of HIPAA laws.
- Ability to establish and maintain a positive and professional relationship with co-workers, clients, and visitors.
- Knowledge and experience of group facilitation and public speaking.
- Demonstrate cultural sensitivity and ability to relate well and effectively in a diverse workplace and with a diverse client population
- Knowledge of current social service resources and delivery systems to meet a wide range of client needs.
- Access to and ability to utilize translation services to clients for whom English is not their first language.
- Ability to organize, efficiently document, and routinely update information in online reporting systems.
- Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint) as well as online platforms such as Zoom, Microsoft Teams and others for online meetings, training and workshops.
- Requires access to and use of an automobile for timely provision of community-based services.
- Ability to work in person Monday-Friday with some occasional evening/weekend hours required.
Education
- Bachelor’s Degree in Social Work or related field of study
Experience
- 1 – 3 years of professional experience
Physical Requirements
- Must be mobile and able to bend, stoop and moderately life (up to 30 pounds)
- Must be able to sit/stand at a workstation or computer screen for up to 2 hours at a time.
- Must be able to sit/stand at both indoors and outdoors events for up to 2 hours at a time.
