POSITION DESCRIPTION
JOB TITLE: Social Work Supervisor

REPORTS TO: Direction from Program Director
DIRECT REPORTS: Case Management Team
MAJOR RESPONSIBILITIES:
Women of Hope-Lombard is a residential Safe Haven serving up to 16 young adults primarily 18-24 years of age. The young women have a history of experiencing homelessness and a mental health diagnosis.
The primary responsibility of the Social Work Supervisor is to oversee a small case management team and provide supportive direction. To assist the Program Director in ensuring effective and efficient functioning of the program. Ensure all residents receive proper supports and certify services are delivered in accordance with DBHID requirement and City regulations. Provide community outreach to develop and maintain client census. Ability to lead a team to accomplish agency strategic goals, program goals and employee evaluation goals. In the absence of the program director, the social work supervisor oversees the program.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Provide weekly supervision to case management team.
- Monitor and document training needs of case management team.
- Provide annual evaluations of case management team.
- Assist case management team to ensure residents housing plans are up to date.
- Review documentation of case records and Medication (MARS) tracking for accuracy.
- Available for On-Call Medication issues.
- Review case notes on a weekly basis and ensure case notes are properly documented using D.A.P. (Data, Assessment, and Plan) format.
- Review daily logs to ensure proper procedures and reports are accurate.
- Ensure accurate Admission and Discharge of residents.
- Submit Event Forms when appropriate (e.g. Admission, Discharge, AWOL, etc.).
- Maintain small caseload (3 residents).
- Engage residents and establish positive rapport.
- Participate in monthly residents meeting.
- Assist with lunch times needed for Mercy Café Day program.
- Assist with any case management needs of Mercy Café Day program.
- Meet weekly with Program Director.
- Attend Concurrent Reviews with Program Analyst.
- Quarterly review of medication distribution procedure with staff and med observation semi-yearly.
- Assist Program Director in preparation of audits and/or inspections of the site, programs and/or case records.
- Ensure program meets all agency, City and Funder requirements.
- Participate in monthly staff meetings, weekly supervision, training, and meetings required by both CSS and Funders.
- Work collaboratively with other community organizations to promote program services.
- Other tasks and duties as assigned.
REPRESENTATIVE KNOWLEDGE/ SKILLS/ ABILITIES:
- Ability to live, implement, and communicate the values and mission of the agency.
- Requires minimum MSW or related degree.
- Progressive leadership skills
- Experience and knowledge of women who have a history of homelessness, trauma, mental health and /or substance use.
- CPR and First Aid Certification.
- Medication Certification.
- Ability to maintain a professional partnership with other community organizations.
- Strong written and verbal communication skills.
- Valid Driver's license preferred.
